To make things easier, we offer the option to complete your new patient forms online. Simply reach out to us to schedule your appointment, and we’ll create a profile for you in our system. Once your profile is set up, you’ll be able to access our online portal to fill out the necessary forms using either your phone number or email address.

Forms
To ensure we can provide you with the best care, there are a few forms we need you to complete before your appointment. These forms are available for online submission, or if you prefer, you can fill them out in the office by arriving 15 minutes early. Our New Patient Coordinator will be happy to assist you.
What to Bring?
Please bring your ID, insurance card, and any x-rays or dental records from your previous dentist to your first appointment. These records will help us better understand your oral health history.
X-Rays
To get a clear picture of your oral health, your hygienist or assistant will take digital x-rays of your mouth. These are important for your dentist to assess your teeth and gums thoroughly. If you have recent x-rays, feel free to bring them along.
Cleaning
If you’re not visiting for an emergency, our hygienist will provide a professional teeth cleaning. During this time, we will also review your medical history and address any concerns you may have about your oral health.
Dr. Exam
After your cleaning, our dentist will perform a comprehensive exam of your teeth and gums, reviewing any x-rays taken. He will discuss your oral health, answer any questions, and provide recommendations for any further treatments if necessary.
